Getting Started

 Appointment Times

At AICC, we offer day, evening and weekend appointments, depending upon provider, in order to best fit your needs.  Please feel free to contact our front office, or the individual provider directly, to learn more about their availability at 770.293.1950. 

 

 

what should i expect on the first appointment?

Please arrive 15 minutes early for your first appointment. Please bring your forms (Link) from this website to your first appointment at the appropriate office location. We will also have a few additional forms for you to complete upon your arrival. The first session will be used to gather information to best help understand your needs and goals for therapy and to collaboratively plan for the therapeutic work at AICC.

 

 

what paperwork should i complete?

It can be very helpful to complete these forms prior to your first appointment. If you are coming as a couple, please have each adult complete the Adult Intake Form. For families, please complete an information form for each member of the family participating. Please complete the Release of Information form if there is another professional you would like your therapist to collaborate with for your most comprehensive care. 

 

 
 

 

use of insurance

Several of our providers accept insurance. For all other providers and networks, we can provide you with the appropriate documentation to submit towards your out-of-network benefits. Once your benefits are met, your insurance company will reimburse you at their contracted rate. Our Insurance Benefits Form will help guide you with the necessary questions and codes to check your benefits prior to beginning counseling. Our front office, or your provider, will be happy to help you with this process. 

 

 

cancellation policy

Please note that we have a 48-hour cancellation policy. Appointments cancelled within 48 hours of the scheduled appointment may result in charges.
